Now is the time to submit your polka video to be placed on the ballot for the USPA Favorite Polka Video of 

2018! This award category is open to every USPA member.

Guidelines for Entries:

• Each entry must have the appropriate rating of "G" and pertain to Polish polkas.
• The video must be of a reasonable length (approximately 3 minutes) and limited to one song.
• Video must have been recorded between January 1, 2018 through December 31, 2018.
• Each entry submitted must include a title, location, the date recorded and the name of the member who recorded it.
• In an attempt to simplify the admission process and encourage participation this year, each entry must be posted on YouTube. This will have the added benefit of providing more exposure to our music.
• Please provide the code to be used to embed your YouTube video, so that it can be streamed from our website.
• Video information can be submitted in two ways:

• In order to be placed on the ballot, all entries must be received no later than Friday, January 25, 2019
• Please be sure to include your name and a telephone number or email address where we can contact you.
